氯吡格雷联合阿司匹林对冠心病心绞痛患者心功能及凝血功能的影响(1)
[摘要] 目的 觀察氯吡格雷联合阿司匹林治疗冠心病心绞痛的临床效果。 方法 选取我院2014年1月～2016年6月收治入院的冠心病心绞痛患者200例作为研究对象，根据治疗方法不同分为两组，每组100例。两组均在常规治疗基础上行针对性治疗，对照组采用口服阿司匹林肠溶片治疗，观察组采用阿司匹林联合氯吡格雷片治疗。观察两组临床疗效、治疗前后心功能指标、凝血指标以及不良反应发生情况。 结果 观察组患者临床总有效率93.0%，明显高于对照组75.0%，且两组比较差异有统计学意义(P<0.05)；观察组治疗后心功能指标LVEF、SV、CO显著高于对照组(t=2.475、2.397、3.052，均P<0.05)；对照组和观察组患者治疗后FIB水平与治疗前差异均有统计学意义(P<0.05)；观察组治疗后血浆FIB水平与对照组治疗后差异有统计学意义(P<0.05)；两组治疗过程中对照组发生胃黏膜出血4例(4.0%)，而观察组发生5例(5.0%)，两组患者不良反应发生率差异无统计学意义(P>0.05)。 结论 氯吡格雷联合阿司匹林能够通过更加有效抗血小板聚集及抑制凝血功能达到快速、高效抗栓作用，提高冠心病心绞痛治疗的临床疗效及心功能，且短期使用安全性良好。

[Abstract] Objective To observe the clinical effect of clopidogrel combined with aspirin in treatment of angina pectoris. Methods 200 cases of angina pectoris of coronary heart disease cured in our hospital from January 2014 to June 2016 were selected as research subjects.They were divided into two groups，according to the different methods with 100 patients in each group.All patients in the two groups were treated with the conventional treatment based on the uplink targeted therapy，patients in the control group were treated with oral Aspirin treatment，and patients in the observation group were treated with aspirin and clopidogrel tablets.Clinical efficacy，cardiac function before and after treatment，coagulation and adverse events of the two groups were compared. Results The clinical total efficiency of observation group was 93.0%，which was significantly higher than that of control group with 75.0%，and the difference was statistically significant (P<0.05).Heart function parameters were observed after treatment group，LVEF，SV，CO were significantly higher that of control group(t=2.475，2.397，3.052，all P<0.05).The difference of FIB level of observation group and control group before treatment were statistically significant(P<0.05).The difference of plasma FIB level of observation group and the control group after treatment was statistically significant(P<0.05).During the treatment，there were 4 patients(4.0%) of gastric bleeding occurred in the control group，and 5 patients(5.0%) in observation group.There was no statistical significance on adverse reactions rate between the two groups(P>0.05). Conclusion Clopidogrel combined with aspirin can more effectively inhibit platelet aggregation and coagulation functions to achieve rapid，efficient antithrombotic effect，and improve the clinical efficacy of the treatment of angina pectoris and heart function.Its short term use has good security., 百拇医药(朱解方 刘央 韩世华)
