无创正压通气治疗稳定期COPD合并慢性Ⅱ型呼吸衰竭患者的疗效分析(1)
[摘要] 目的 探讨无创正压通气治疗稳定期慢性阻塞性肺疾病(COPD)合并慢性II型呼吸衰竭患者的疗效。 方法 将2015年2月~2016年3月72例稳定期COPD合并慢性呼吸衰竭患者随机分为对照组与实验组,每组36例。对照组采用经鼻导管吸氧,实验组采用氧疗+BiPAP呼吸机行无创正压通气治疗。比较两组血气分析指标、心率和呼吸频率。 结果 通气治疗后实验组PaCO2为(54.93±1.86)mm Hg、pH为(7.38±0.03),较对照组的(63.82±2.30)mm Hg、(7.28±0.04)改善更佳(P<0.05);两组PaO2、SaO2差异不大。治疗后两组急性加重次数与6min步行距离均有显著改善(P<0.05);治疗后实验组急性加重次数与6min步行试验结果较对照组明显改善(P<0.05)。 结论 无创正压通气治疗稳定期COPD合并慢性Ⅱ型呼吸衰竭患者效果良好,可及时改善患者通气,缓解呼吸肌疲劳减少机体耗氧量,增加活动耐力,以及减少急性加重次数,具有积极的临床意义。

[Abstract] Objective Toexplore the curative effect of noninvasive positive pressure ventilation in treatment of patients with stable chronic obstructive pulmonary disease(COPD) complicated with chronic type II respiratory failure. After treatment,times of acute exacerbation and 6-minute walking distance in two groups were both significantly improved(P<0.05).After treatment,times of acute exacerbation and 6-minute walking distance of the experimental group were more significantly improved than those of the control group(P<0.05).
