蕲蛇酶注射液治疗脑梗死的系统评价和Meta分析(1)

     【摘要】目的 系統评价蕲蛇酶注射液治疗脑梗死的有效性和安全性。方法 检索中国知网、中国生物医学文献数据库、重庆维普中文科技期刊数据库、万方数据库、EMbase、PubMed、Cochrane Library等数据库,检索时限为建库至2019年7月1日,收集蕲蛇酶注射液治疗脑梗死的随机对照试验(RCT)。由两名研究者独立筛选文献,进行数据提取、偏倚风险和质量评估,采用RevMan 5.3软件对纳入的文献进行数据的Meta分析。结果 共纳入10篇文献,样本总量为992例,试验组512例,对照组480例,纳入研究总体质量不高。Meta分析结果显示:蕲蛇酶+常规治疗脑梗死在总有效率上优于常规治疗(OR=3.88,95%CI[2.52~5.97],P<0.00001);蕲蛇酶+常规治疗脑梗死在NIHSS评价方面优于常规治疗(MD=4.03,95%C[I3.04~5.02],P<0.00001);蕲蛇酶+常规治疗在Barthel指数方面优于常规治疗(MD=20.52,95%CI[13,37~27.68],P<0.00001);纳入研究报告的不良反应均为轻度不良反应(OR=1.42,95%C[I 0.76~2.67],P=0.27),停药进行对症处理后均好转。结论 蕲蛇酶联合常规治疗脑梗死具有一定的疗效,且不良反应较少,应用较为安全。但鉴于其纳入研究证据质量等级不高,对结论的可靠性会造成影响,应谨慎使用研究结论;且研究结论仍需大样本、多中心、设计科学严谨、高质量的随机对照试验加以验证蕲蛇酶注射液治疗脑梗死的疗效和安性。

    Systematic review and Meta-analysis on randomized controlled trials on effectiveness and safety of aconiase Injection in treatment of cerebral infarction

    【Abstract】Objective To evaluate the efficacy and safety of python enzyme injection in the treatment of cerebral infarction.Methods Search the database of China Knowledge Network,China Biomedical Literature Database,Chongqing Weipu Chinese Science and Technology Journal Database,Wanfang Database,EMbase,PubMed,Cochrane Library,etc.The search time limit is to build the library until July 1,2019,collecting venom enzymes.A randomized controlled trial(RCT)of injections for the treatment of cerebral infarction.The literature was independently screened by two investigators for data extraction,risk of bias,and quality assessment.Meta-analysis of the included literature was performed using RevMan 5.3 software.Results A total of 10 articles were included.The total sample size was 992,the experimental group was 512,and the control group was 480.The overall quality of the included study was not high.Meta-analysis showed that python enzyme+conventional treatment of cerebral infarction was superior to conventional treatment in total effective rate(OR=3.88,95%CI[2.52~5.97],P<0.00001);python enzyme+conventional treatment of cerebral infarction NIHSS evaluation was superior to conventional treatment(MD=4.03,95%CI[3.04~5.02],P<0.00001);python enzyme+conventional treatment was superior to conventional treatment in Barthel index(MD=20.52,95%CI[13],37~27.68],P<0.00001);all adverse reactions included in the study were mild adverse reactions(OR=1.42,95%CI[0.76~2.67],P=0.27),and all of them improved after symptomatic treatment..Conclusion Aphid enzyme combined with conventional treatment of cerebral infarction has certain curative effect,and has fewer adverse reactions and is safer to apply.However,in view of the fact that the quality of the evidence included in the study is not high,the reliability of the conclusion will be affected.The research conclusions should be used cautiously;and the conclusions of the study still need to be verified by large-sample,multi-center,scientifically rigorous,high-quality randomized controlled trials.The efficacy and safety of snake enzyme injection in the treatment of cerebral infarction., http://www.100md.com(乔瑞英)
