急危重症床旁超声检查的快速评估及应用研究(1)
【摘要】目的 ：探析床旁超声快速评估急危重症患者的病情及其诊断方面的临床价值。方法：本次研究回顾性分析我院2012年10月~2014年3月期间行床旁超声检查的528例患者的临床资料，对比分析床旁超声诊断结果和临床诊断的差异，探析床旁超声检查在急危重症患者诊断中的应用价值及其思路分析。结果 ：本组528例急危重症患者经X线和CT等影像学检查、实验室检查、临床穿刺、手术病理证实为心血管疾病186例，床旁超声诊断符合181例，诊断符合率为97.31%；腹部急危重症342例，床旁超声诊断符合321例，诊断符合率为93.86%；床旁超声总体符合率为95.08%，漏诊率和误诊率为4.92%。结论： 床旁超声快速评估急危重症患者的病情能为临床医师早期确诊提供重要的参考信息，提高早期诊断符合率，有利于控制医疗风险，具有极其重要的临床应用价值和意义，值得广泛推广和应用于临床急危重症患者的早期诊断过程中。

[Abstract] Objective: analysis of the clinical value of bedside ultrasound rapid assessment of acute and critically ill patients and its diagnosis. Methods: To review the research analysis of our hospital in 2012 October ~2014 year in March during the clinical data of 528 cases of bedside ultrasonography in patients, to analyze the difference of bedside ultrasound diagnosis and clinical diagnosis, analysis of bedside ultrasound application. In the diagnosis of patients with acute and severe in value and its analysis. Results: in this group, 528 cases of acute and critically ill patients by imaging X-ray and CT examination, laboratory examination, clinical puncture, operation and pathology of cardiovascular disease in 186 cases, bedside ultrasound diagnosis in 181 cases, the diagnostic coincidence rate was 97.31%; 342 cases with abdominal emergency bedside ultrasound diagnosis, 321 patients with, the diagnostic coincidence rate was 93.86%; bedside ultrasound overall compliance rate was 95.08%, the rate of missed diagnosis and misdiagnosis rate was 4.92%. Conclusion: rapid assessment of emergency bedside ultrasound in patients with critical illness can provide important reference information for clinical early diagnosis, early accurate diagnosis, to control the medical risks, its clinical application has very important value and significance, early diagnosis and worthy of extensive promotion and application in clinical acute and critically ill patients.

超声检查作为临床广泛应用的一种影像学辅助检查方法，其具有无创、操作简单且有效，已经被广泛应用于医疗卫生科学的各个领域，其具有实时诊断、不受断面限制的特征，已经被应用于便捷、精准诊断患者疾病的一项重要辅助工作，随着科技不断发展，超声技术也逐步完善，床旁超声设备不断更新换代，为床旁超声在临床中的应用提供了有利的条件[1]。床旁超声的应用，为患者的诊治赢得了充足的时间，尤其对危急重症患者的诊疗更为显著，目前，床旁超声已经广泛应用于重症监护室、急诊科、手术室等科室的诊断、鉴别诊断、监测以及评估病情等，发挥中极其重要的临床作用[2]。本次研究笔者为进一步探析床旁超声应用于危急重症患者诊断中的价值，现回顾性分析528例行床旁超声检查患者的临床资料，现将结果报道如下。
1 资料和方法

1.1 一般临床资料
本次研究笔者收集我院2012年10月~2014年3月期间收治的528例急诊和住院患者的临床资料，其中住院患者402例、急诊患者126例，男性382例、女性146例，年龄22~78岁，平均年龄51.06±18.48岁。经X线和CT等影像学检查、实验室检查、临床穿刺、手术病理证实为心血管疾病186例，原发疾病：急性心肌梗死、主动脉夹层、急性肺栓塞、心包积液，(详见表1)；腹部急危重症342例，原发疾病：肝破裂、脾破裂、肾损伤、空腔脏器损伤、急性胰腺炎、前置胎盘、异位妊娠、黄体破裂，(详见表2)。
1.2 仪器和方法
床旁超声使用阿洛卡和开立便携式彩色多普勒超声诊断仪，心脏超声检查探头频率设置为2.5MHz，腹部超声检查探头频率设置为3.5MHz。在检查过程中依据患者病情状况确定合理的检查体位，按照临床诊断要求尽可能做到迅速、多切面以及多部位扫查。检查完毕后立即书写超声检查报告，直接报告检查的结果。
1.3 统计学处理
本次研究笔者分别统计心血管疾病、腹部疾病床旁超声诊断情况，采用SPSS18.0软件对所得的数据进行统计学分析，计数资料采用百分率表示，χ2检验，检验标准P< 0.05则具有统计学意义。, 百拇医药(刘栋文)
