双侧子宫动脉结扎术联合水囊压迫治疗前置胎盘剖宫产术中及产后出血的临床效果观察(1)
【摘要】目的:探討双侧子宫动脉结扎术联合水囊压迫在对前置胎盘剖宫产孕妇产后出血的止血效果。方法:选取 2017 年1月至2018年10月本院收治的因前置胎盘而接受剖宫产手术,且术中或术后发生产后出血的产妇作为观察组研究对象,对其采取双侧子宫动脉结扎术联合水囊压迫止血;并从2015年1月至2016年12月同样发生产后出血且接受宫腔纱布填塞止血法的孕妇中随机选出58例作为对照组研究对象,比较两组产妇的止血效果和并发症发生情况。结果:1)观察组产妇术中、产后24h平均出血量均显著低于对照组(P<0.05);观察组产妇的宫腔填塞物放置时间也显著短于对照组产妇(P<0.05)。2)观察组孕妇中,有1例最终因持续出血而接受子宫切除术,对照组则有9例行子宫切除术,观察组孕妇子宫切除率显著低于对照组(P<0.05)。3)两组产妇中产后发热、子宫复旧不良等并发症的发生率无显著差异(P>0.05)。结论:双侧子宫动脉结扎术联合水囊压迫止血对于前置胎盘剖宫产后产后出血具有更为良好的止血效果,可以显著减少产后出血量和行子宫切除术的风险,且增加感染风险,也不影响子宫复旧。

[Abstract] Objective: To investigate the hemostatic effect of bilateral uterine artery ligation combined with water capsule compression on postpartum hemorrhage of caesarean section women with placenta previa. Methods: From January 2017 to October 2018, the pregnant women who underwent caesarean section due to placenta previa and had postpartum hemorrhage during or after operation were selected as the observation group. Bilateral uterine artery ligation combined with water sac compression was used to stop bleeding. From January 2015 to December 2016, 58 pregnant women who also had postpartum hemorrhage and received uterine gauze packing and hemostasis were randomly selected as the control group. The hemostatic effect and complications of women. Results: The average blood loss during operation and 24 hours postpartum in the observation group was significantly lower than that in the control group (P<0.05); The time of intrauterine tamponade placement in the observation group was also significantly shorter than that in the control group (P<0.05). Among the pregnant women in the observation group, 1 case received hysterectomy due to continuous bleeding, while 9 cases in the control group received hysterectomy. The rate of hysterectomy in the observation group was significantly lower than that in the control group (P<0.05). There was no significant difference in the incidence of complications such as postpartum fever and uterine dysplasia between the two groups (P>0.05). Conclusion: Bilateral son Uterine artery ligation combined with water sac compression hemostasis has a better hemostatic effect on postpartum placenta hemorrhage after cesarean section. It can significantly reduce postpartum bleeding and the risk of hysterectomy, and increase the risk of infection. Also does not affect the uterine restoration, worthy of clinical promotion.
