银川市社区老年人医养结合服务需求及影响因素研究(1)
【摘 要】目的：了解银川市社区老年人医养结合服务需求及影响因素，为银川市更好地开展医养结合服务提供一定的参考依据。方法：采用便利抽样法，于2019年1至2019年3月选择银川市3个社区卫生站120例符合纳入排除标准的社区老年人为研究对象，采用《社区老年人医养结合服务需求调查问卷》进行问卷调查。结果：社区老年人医养结合服务需求率为92.7%。多元线性回归结果显示影响社区老年人医养结合服务需求的因素包括婚姻状态、居住状况、家庭月收入、患慢性病情况、半年就诊次数和医养结合知晓度等。结论：银川市社区老年人医养结合需求较高，其中社会福利类需求最高。建议社区卫生机构以社区老年人医养结合服务需求为导向，更好地开展社区老年人医养结合工作。

[Abstract] Objective：To identify the needs and influencing factors of medical and elderly care services for the community elderly in Yinchuan City， and to provide a reference for Yinchuan City to better deliver medical care combined services. Methods： By convenience sampling method， 120 community seniors from three community health stations in Yinchuan City were selected， and the questionnaire survey was conducted using the questionnaire of community elderly medical care combined service needs. Results： The demand rate of medical care combined services for the elderly in the community was 92.7%. The results of multiple linear regression showed that the factors affecting the demand for medical care combined services in the community include marital status， living conditions， family monthly income， chronic diseases， half-year visits and awareness of medical and elderly care . Conclusion： The demand for medical care combined services in the elderly in Yinchuan City is high， and the demand for social welfare is the highest. It is suggested that community health institutions should be guided by the needs of the community's elderly medical care and combined services， and better carry out medical care combined services for the elderly in the community.

自2000年我国进入老龄化社会后，老年人口不断增多。截至2016年我国60岁以上老年人口已达2.3086亿人，占总人口的16.7%;65岁及以上人口1.5003亿人，占总人口的10.8%[1]。人口老龄化给我国医疗与养老事业带来了严峻挑战，医养衔接程度不高、服务水平不高、工作机制不健全和政策保障不足等问题日益凸显，而医养结合这一政策的提出成为解决医疗与养老问题的新曙光。2015年国务院办公厅转发《关于推进医疗卫生与养老服务相结合的指导意见》指出，为积极应对老龄化，应全面推进医疗卫生与养老服务相结合，力图打通健康养老的“最后一公里”[2]。本研究旨在通过了解银川市社区老年人医养结合服务需求现状，分析影响社区老年人医养结合服务需求的因素，为银川市更好地开展社区老年人医养结合服務工作及制订医养结合服务内容提供一定的参考依据。
1 资料与方法
1.1 调查对象
采用便利抽样方法，抽取银川市3个社区卫生服务站120名老年人进行问卷调查。纳入标准：1)社区常住居民(居住时间≥6个月);2)意识清楚，与研究人员交流无障碍者;3)知情同意且自愿接受本次调查者。排除标准：因精神或认知障碍等因素无法正常沟通交流者。
1.2 研究方法
1.2.1 调查工具 采用一般资料调查表、社区医养结合型养老服务(MNCS)需求调查表进行问卷调查。
1)一般资料调查表：包括年龄、婚姻状况、居住状态、家庭月收入、患慢性病情况、医养结合知晓度等19个条目。
2)社区医养结合型养老服务(MNCS)需求量表采用由田甜等学者研究设计的量表，分为生活照料类、医疗服务类、精神需求类、社会参与类、法律维权类和社会福利类6个维度共50个条目。每个条目采用Likert 5级评分法进行赋值，依次为：不需要=1分，不太需要=2分，偶尔需要=3分，需要=4分，十分需要=5分。以5分值中的均值2.5分为依据，若某个维度的实际均分高于此分，说明老年人对该维度的需求水平较高。, http://www.100md.com(仇艳敏 刘国莲 何旭文 牛萌 刘赟赟 郑连花)
