The department of cardiovascular surgery, Xijing Hospital affiliated to the Fourth Military University successfully treated a case of dissecting aneurysm using the technique of intra-lumen isolation by membranous pedicled stent. Literature search revealed that this was the first case of the kind treated in the Western region of China.
The patient was46 years of age, a native of Linfeng, Shanxi province, had severe chest pain because of a dissecting aneurysm. His blood pressure was as high as 220/135 mmHg, accompanied with severe headache. He was treated in quite a few hospitals in his own locality and Taiyuan municipality without any effect and then came to Xijing Hospital. After admission, massive dosage of anti-hypertensive agent was given without any effect. For the salvage of the patient Dr. Cai Zhenjia and associate professor Cui Qin of the department of cardiovascular surgery under the directions of Professor Cai Zhengjie, chief surgeon of the department and professor Zhang Weida , performed a small incision of 3-5 cm with guidance of the x-ray and a membranous stent was inserted to block the orifice of the dissecting aneurysm. Re-examinstion with contrast medium revealed complete isolation of the dissecting aneurysm. Blood pressure returned to normal and the symptoms of headache, and chest pain disappeared. Patient was allowed ambulation the next day after surgery and was discharged from the hospital 5 days later in good condition.
The procedure is easily accepted by the patients and is done with a minimal incision for the treatment of aneurysm mostly used internationally. In China this procedure is only performed in the individual major hospitals in Beijing and Shanghai; in Xijing hospital the intralumenal isolation procedure has been carried out for the thoracic and the abdominal aneurysms.
